License by Mail
All licenses expire on December 31st regardless of the date of purchase. Payment for your license is due by February 28th.
By ordinance, a late fee is added to each license not purchased within 30 days of the expiration of the prior license.
Please mail a description of your pet and proof of spay/neuter, along with your check or money order payable to "City of Lakewood Animal Licensing" to the address below. Please do not send cash.

- Proof of spay/neuter, such as a signed statement from a veterinarian, is required if the pet has been altered since the last licensing period and you are applying for the lower spayed/neutered license fee.
- All pets are required to have a current rabies vaccination. A copy of a current rabies vaccination certificate is requested for each pet.
Note: Please send legible copies of your paperwork, as they will not be returned with your tag and receipt.
